Introduction

The eMedica ePHR (the "Service") is a health management tool from eMedica Technologies Inc. (“eMedica”).  In this document, "We" or "Our" refers to eMedica. The Service features certain health content and guidance and is hosted by eMedica. It is important for you to know how the information that you submit, review and edit in the Service is handled while you use this site.

eMedica provides a platform for the secure storage of, and access to one’s personal health information. In the course of our activities in providing this service, eMedica may collect, use and disclose Personal Information. To demonstrate our commitment to your privacy, eMedica has prepared this policy statement to inform you how we endeavor to ensure the privacy and confidentiality of your Personal Information.

 

Collection of Personal and Usage Information

Use of the Service and by extension, the Service's tools and features e.g. the eMedica Card (the “Card”) requires input of personal data from or on behalf of an eMedica card owner.  That data is stored on the eMedica Card and can be accessed via a corresponding, secure eMedica account.  The Service does not permanently store any personal data but does provide a feature that allows data to be backed up to the eMedica secure servers.  A copy of one’s static health information is created on the secure servers that allows for information to be viewed through a secure web connection or from a smartphone or tablet device (iPhone®; iPad®; Android® device).  Information can be reviewed and edited in the Service, and any changes will be saved to one’s eMedica account. All of this information, when it is held by or transmitted to the Service outside of the ePHR, is referred to as "Personal Information."

We may collect information about your interaction with the Service. For example, we may use Web site analytics tools on the Service to retrieve information from your browser, including the site you came from, the search engine(s) and the keywords you used to find the Service, the pages you view within the Service, and your browser's width and height.  We also may use technologies, such as cookies and Web beacons (described below), to collect information about the pages you view, the links you click and other actions you take on the Service. Additionally, we collect certain standard information that your browser sends to every Web site you visit, such as your IP address, browser type and language, access times, and referring Web site addresses. This information is referred to as "Usage Information." 

 

How your Personal Information is stored

The Service stores Personal Information as necessary to provide you with the Service. The Service does maintain a permanent copy of your backed up health information until you delete the backup, or discontinue the service. 
All of the Personal Information entered, edited or viewed in the Service is held only temporarily by the Service.  Information entered or edited in the Service is saved upon logging off from the Service and accepting the "Save a backup" option. The Service is only available to holders of an eMedica account.

All Personal Information entered into the Service is secured and encrypted using Secure “Socket Layer” technology during transmission using the Internet. All Personal Information is saved in an encrypted format on the eMedica Card.  Personal Information and Usage Information are stored on servers located in the United States, or Canada in accordance with privacy legislation.

 

What Personal Information Does eMedica Hold?

It is important for readers of this Policy Statement to note a distinction between "possession" and "custody". When you use eMedica’s services and access your personal health information, you are the custodian of that personal health information. eMedica has possession of that information only if you have backed up your information to our secure servers. eMedica has no independent interest in such information. eMedica is not a custodian of this information and is not a "health information custodian" within the meaning of health privacy legislation.

eMedica does have custody of other Personal Information. This Personal Information consists of identifying and billing-related information as well any information about our interactions with you while you are an eMedica client or service provider.  

 

How your Personal Information and Usage Information are used and shared

Personal Information and Usage Information are used to provide the Service and personalize the eMedica Cardholder’s experience.  We reserve the right to engage other companies to provide services under contract, such as hosting and supporting the operation of the Service. Any Personal Information and or personally identifiable Usage Information that is required to deliver the Service will be limited to basic identifiers such as IP address or e-mail address. Except as set forth in this statement, we will not share your Personal Information and Usage Information in personally identifiable form with any other party without your consent, unless we are required to do so to comply with the law, or to prevent report or investigate illegal activity.  

 

Third party links

In the event that The Service contains links to Web sites operated by third parties, we maintain no control over the privacy policies and practices of such third party sites, and recommend reviewing the privacy policies and terms of use of those sites for more information about the policies applicable to those sites.

 

How we use “cookies” 

"Cookies" are small pieces of information that some Web sites use when visited to store information on a computer's hard drive. Like many other Web sites, the Service uses cookies to provide information relating to the sources of site traffic and to help personalize the user experience.  Collection of this information, however, does not allow for personal identification of any user.


Most Web browsers automatically accept cookies but allow you to modify security settings so that you can approve or reject cookies on a case-by-case basis. If you choose to decline cookies, you may not be able to sign in or use other interactive features of sites and services that depend on cookies, and some advertising preferences that are dependent on cookies may not be able to be respected.

Use of Web beacons

The Service Web pages may contain electronic images known as Web beacons — sometimes called single-pixel gifs — that may be used to assist in delivering cookies and allow for the assimilation of general demographic details of unique web visits.
We may also employ Web beacons from third parties in order to help us compile aggregated statistics regarding the effectiveness of our promotional campaigns or other operations of our sites. We prohibit Web beacons on our sites from being used by those third parties to collect or access your Personal Information.


Complaint process

If you have a complaint or problem related to the Service, e-mails to Customer Support (support@emedicatech.com) will be diligently reviewed and answered. Based on the nature of a complaint, it will be directed to the appropriate Customer Service Department. If an adequate resolution to a privacy-related problem is not received, you may write to the Service's Privacy Officer at:

Changes to our Privacy Statement

We will occasionally update this Privacy Statement. Each eMedica cardholder will be prompted to accept the new privacy statement before the next time a log-in occurs with the Service.
